Through trusted local networks, Horizon delivers faith-based mental health education, reproductive health support, and school-based care to rural and peri-urban communities. We work hand in hand with district hospitals to extend care beyond the clinic walls.

In the DRC, we support high-risk communities through our cross-border health mission, bringing street medicine, chronic illness management, and trauma support to areas affected by instability. Our presence in Goma and neighboring provinces connects us directly to families in crisis.

We serve vulnerable populations in Dar es Salaam and northern regions through pop-up clinics, nutrition programs, and maternal health outreach. Our Saturday outreach in partnership with religious centers provides consistent access to basic care, even in under-resourced coastal communities.

In Kampala and surrounding districts, we operate community health days, hygiene supply drops, and youth outreach through local churches and mosques. Our mobile team brings treatment, medication, and hope directly to urban informal settlements and displaced families.
